NOTICE TO BIDDERS MUNICIPALITY OF MT. LEBANON ALLEGHENY COUNTY, PENNSYLVANIA Sealed Proposals will be received by the Mt. Lebanon Commission at the Municipal Building, 710 Washington Road, Pittsburgh, PA 15228 until 11:00 a.m. prevailing time on March 14, 2014 for the following: 2014 BRICK STREET PAVEMENT REPAIR PROGRAM This project includes the restoration of brick streets throughout the Municipality Proposals shall be delivered in a sealed envelope that is clearly marked on the outside with the appropriate wording "BID FOR THE 2014 BRICK STREET PAVEMENT REPAIR PROGRAM" to the desk of the Director of Public Works, Thomas Kelley, Municipal Building, 710 Washington Road, Pittsburgh, PA 15228 before 11:00 a.m. prevailing time, March 14, 2014 delivered to the 1st Floor Service Center, and the same will be publicly opened and read at that time at the Municipal Building Commission Chambers located at 710 Washington Road, Pittsburgh, PA 15228. Copies of Drawings, Specifications, Instructions to Bidders, General Conditions, Forms of Proposals and Agreement are on file and open to public inspection at the Mt. Lebanon Office of Public Works, 710 Washington Road, Pittsburgh, PA 15228 where sets of said documents may be obtained at no cost. Documents will be available beginning on February 27, 2014. Office hours are Monday through Friday 9:00 A.M. to 4:00 P.M. Prevailing wages will not be required to be paid on this project. YEARLY CONCRETE CURB MAINTENANCE PROGRAM This project includes the removal and replacement of various sizes of concrete straight curb, concrete roll curb, and concrete curb and gutter throughout the Municipality Proposals shall be delivered in a sealed envelope that is clearly marked on the outside with the appropriate wording "BID FOR THE YEARLY CONCRETE CURB MAINTENANCE PROGRAM" to the desk of the Director of Public Works, Thomas Kelley, Municipal Building, 710 Washington Road, Pittsburgh, PA 15228 before 11:00 a.m. prevailing time, March 14, 2014 delivered to the 1st Floor Service Center, and the same will be publicly opened and read at that time at the Municipal Building Commission Chambers located at 710 Washington Road, Pittsburgh, PA 15228. YEARLY ROOT DAMAGED SIDEWALK MAINTENANCE CONTRACT This project includes the removal and replacement of concrete sidewalk damaged by tree roots throughout the Municipality. Proposals shall be delivered in a sealed envelope that is clearly marked on the outside with the appropriate wording "BID FOR THE ROOT DAMAGED SIDEWALK PROGRAM" to the desk of the Director of Public Works, Thomas Kelley, Municipal Building, 710 Washington Road, Pittsburgh, PA 15228 before 11:00 a.m. prevailing time, March 14, 2014 delivered to the 1st Floor Service Center, and the same will be publicly opened and read at that time at the Municipal Building Commission Chambers located at 710 Washington Road, Pittsburgh, PA 15228. YEARLY MAIN ROAD ASSESSMENT SIDEWALK MAINTENANCE CONTRACT This project includes the removal and replacement of concrete sidewalk. Proposals shall be delivered in a sealed envelope that is clearly marked on the outside with the appropriate wording "BID FOR THE YEARLY MAIN ROAD ASSESSMENT SIDEWALK MAINTENANCE CONTRACT" to the desk of the Director of Public Works, Thomas Kelley, Municipal Building, 710 Washington Road, Pittsburgh, PA 15228 before 11:00 a.m. prevailing time, March 14, 2014 delivered to the 1st Floor Service Center, and the same will be publicly opened and read at that time at the Municipal Building Commission Chambers located at 710 Washington Road, Pittsburgh, PA 15228. LEBANON 2014 STREET MAINTENANCE PROGRAM The following streets are included in the project: Pine Avenue Golf Course to Municipal Line Lakemont Drive Greenhurst to Larchdale Newburn Drive Robinwood to Municipal Line Florida Avenue 900 Florida to Cochran Larchmont Road Cochran to Pinetree Dupont Circle Entire Road Sleepy Hollow Road 802 Sleepy Hollow to 477 Sleepy Hollow Major bid items are: Milling of existing materials, leveling with one (1) inch thick bituminous pavement (where applicable), paving fabric where required, 1-1/2" thick minimum bituminous overlay and bituminous binder throughout the Municipality of Mt. Lebanon. Proposals shall be delivered in a sealed envelope that is clearly marked on the outside with the appropriate wording "2014 Street Maintenance Program" to the desk of the Director of Public Works, Thomas Kelley, Municipal Building, 710 Washington Road, Pittsburgh, PA 15228 before 11:00 a.m. prevailing time, March 14, 2014 delivered to the 1st Floor Service Center, and the same will be publicly opened and read at that time at the Municipal Building Commission Chambers located at 710 Washington Road, Pittsburgh, PA 15228. Copies of Drawings, Specifications, Instructions to Bidders, General Conditions, Forms of Proposals and Agreements are on file and open to public inspection at the office of The Gateway Engineers, Inc., 400 Holiday Drive, Suite 300, Pittsburgh, PA 15220 where sets of said documents may be obtained on February 27, 2014, upon payment of $50.00 for the contract. No refund or exchange will be made for the return of any documents. A $10.00 fee will be applied to any specifications which are mailed to bidders. All bidders must be PennDOT pre-qualified and meet the work classification and capacity rating for the types of work specified in the bidding documents. All bids submitted must have the contractor's valid pre-qualification certification attached to the outside of the bid or it will be rejected. A price adjustment of bituminous materials will be permitted in accordance with section 110.04 of Penn- DOT Publication 408. The Contractor shall calculate and submit their bid based on the current asphalt index for the month of the bid opening. The Contractor shall strictly adhere to the Contract schedule for executing the Contract documents, starting and finishing the project. No additional compensation will be paid if there is a delay in executing the Contract or starting construction without a time extension granted via formal change order issued by the Municipality. Prevailing wage rates will be required on the street maintenance contract. MT. LEBANON 2014 CRACK SEALING & UTILITY PATCH PROGRAM This project includes the crack sealing of various streets and utility patch milling/overlays throughout the Municipality of Mt. Lebanon. Proposals shall be delivered in a sealed envelope that is clearly marked on the outside with the appropriate wording "BID FOR THE MT. LEBANON 2014 CRACK SEALING & UTILITY PATCH PROGRAM" to the desk of the Director of Public Works, Thomas Kelley, Municipal Building, 710 Washington Road, Pittsburgh, PA 15228 before 11:00 a.m. prevailing time, March 14, 2014 delivered to the 1st Floor Service Center, and the same will be publicly opened and read at that time at the Municipal Building Commission Chambers located at 710 Washington Road, Pittsburgh, PA 15228. LEBANON 2014 STREET RECONSTRUCTION PROGRAM The following streets are being considered: Boxwood Drive Driftwood Drive to Arrowood Drive Crescent Drive Beadling Road to Inglewood Drive Crestvue Manor Drive Washington Road to Lebanon Hills Drive Dillon Drive 124 Dillon Drive to Terminus Jefferson Drive Parkway Drive to 354 Jefferson Drive Oakwood Drive Sunridge Drive to Municipal Line Osage Road 770 Osage Road to 716 Osage Road Rocklynn Place Rock Haven Lane to Mt. Lebanon Boulevard Twin Hills Drive Ridgefield Avenue to N. Meadowcroft Avenue Woodland Drive Ridgeview Drive to 107 Woodland Drive Major bid items are: New Bituminous Pavement (1 ½" Superpave 9.5 mm Bituminous Wearing Surface, 1 ½" Superpave 19.0 mm Bituminous Binder, Superpave 25.0 mm Bituminous Binder, and Superpave 25.0 mm Bituminous Concrete Base Course); removal and reconstruction of existing base, removal of concrete pavement, construction of roof drain collectors, removal and reconstruction of concrete curbs, handicapped ramps, construction and reconstruction of storm inlets, storm and sanitary sewer manholes and installation of new storm sewer pipe. Proposals shall be delivered in a sealed envelope that is clearly marked on the outside with the appropriate wording "2014 Street Reconstruction Program" to the desk of the Director of Public Works, Thomas Kelley, Municipal Building, 710 Washington Road, Pittsburgh, PA 15228 before 11:00 a.m. prevailing time, March 14, 2014 delivered to the 1st Floor Service Center, and the same will be publicly opened and read at that time at the Municipal Building Commission Chambers located at 710 Washington Road, Pittsburgh, PA 15228. Copies of Drawings, Specifications, Instructions to Bidders, General Conditions, Forms of Proposals and Agreements are on file and open to public inspection at the office of The Gateway Engineers, Inc., 400 Holiday Drive, Suite 300, Pittsburgh, PA 15220 where sets of said documents may be obtained on Friday, February 28, 2014, upon payment of $50.00 for the contract. A $10 mailing fee will be applied to any specifications which are mailed to bidders. No refund or exchange will be made for the return of any documents. All bidders must be PennDOT pre-qualified and meet the work classification and capacity rating for the types of work specified in the bidding documents. All bids submitted must have the contractor's valid pre-qualification certification attached to the outside of the bid or it will be rejected. Prevailing wage rates will be required to be paid on the street reconstruction contract. A price adjustment of bituminous materials will not be permitted. Performance and Labor and Material Payment Bonds, along with Public Liability and Property Damage Certificates of Insurance in the amounts specified, as well as Certificates of Workman's Compensation must be filed with the executed Agreement. Proposals to receive consideration must be accompanied by a Certified Check or Bidder's Bond from a Surety Company authorized to do business in Pennsylvania, made to the order of Municipality of Mt. Lebanon in an amount equal to ten percent (10%) of the total amount of the Proposal as a guarantee that, if the Proposal is accepted, the successful Bidder will enter into an Agreement within 15 days after Notice of the Award of the Contract. The Commission of Mt. Lebanon, Pennsylvania reserves the right to waive any formalities or to reject any or all bids. If more time is necessary for the ascertainment of the lowest responsible bidder, the Commission of Mt. Lebanon reserves the right to award the contract at a later date, but not exceeding sixty (60) days from the date bids were submitted, and such date will be publicly announced immediately after the bids are opened.
